Social Prescribing is a project of MVSC working in partnership with the Merton Clinical Commissioning Group, Merton Council, Merton GP Practices and the voluntary and community sector.
The service supports people aged 18 and over in identifying what they think are their main needs and goals and works up a plan to meet these.
